When the document opens in your browser, you will be
able to fill out the form at the bottom of the document
using drop down menus. The fields without drop
down menus are manual text entry fields. Just
click on the field and type the information requested.
If
you click on the "Highlight Fields" option near the top
of the document, it will make it easier for you to see
what needs to be filled out. There are also
prompts on some of the fields where information is
required. |
